- After a serious near miss, an injury, or a RIDDOR.
- Interviews, the immediate cause, and the underlying one.
- Enough to prepare the RIDDOR. We do not file it with the HSE.
- Actions back onto the record, with owners, before the site moves on.

- The photos, the witnesses, and the causes sit on the incident.
- The RIDDOR fields are prepared. Sending it stays with you.
- The actions are open before the site has moved on.
